Ischaemic Heart Disease

INIMED 360° characteristics
  • Exclusively mammary artery bypasses, regardless of the pontable vessels..
  • Excellent results on patients that suffer of diabetes.

The ischaemic heart disease represents the heart muscle (myocardium) – related distress caused by the narrowing (stenosis) or blockage (occlusion) of the coronary arteries (heart arteries). The main cause of the coronary distress is the atherosclerosis, which consists of the accumulation of cholesterol and other cellular elements on the inner artery wall, producing so called atheroma plaque. It narrows and it eventually blocks the artery, by blocking the blood flow towards the myocardium. Myocardial infarction (or heart attack) occurs when a part of the heart muscle is completely lacking blood and, therefore, the cardiac cells of the affected area die.

Treatment

  • Risk factors adjustment
  • Medication-assisted treatment
  • Interventional treatment – coronary angioplasty with stent insertion – in case of stenosis with a limitation of more than 50% – it consists of minimally invasive endovascular desobstruction and dilatation of coronary arteries. A tube called stent (metal cylinder) is inserted in the stenosis location and it is used to keep a proper diameter of the artery.
  • Surgical treatment – Aorto-coronary bypass, when more coronary arteries are affected by diffuse atherosclerotic plaques. The surgery consists of fitting by-passes which bypass the stenosis or occlusion-affected areas of the coronary arteries. Pieces of leg veins (saphenous vein) or pieces of arteries which irrigate less important areas (mammary arteries, radial arteries) are used. Arteries keep their permeability longer, comparing to venous grafts.

The natural evolution without surgical treatment

Nowadays, there is no doubt that the cardiovascular diseases represent the most common mortality cause, more than 50 % of the cases, out of which 80- 90 % are due to ischaemic heart disease and myocardial infarction.
In lack of the adjustment of the risk factors, of the medication-assisted and surgical treatment, the coronary atheromasia invariably progresses from stenosis to occlusion, when the myocardial infarction occurs.
